Abstract

Purpose: Technology recently has been developed to enable use of the CO2 laser in ablation mode and cutting mode together. In this connection, we would like to present our experience with CO2 laser for the treatment of benign periocular lesions, for cosmetic eyelid surgery and eyelid malpositions surgery. Material and Methods: 170 periocular surgical operations have been done in the Istanbul University Cerrahpasa Medical Faculty Ophthalmology Department, Oculoplastic division. Blepharoplasty operation, periocular rejuvenation “resurfacing”, ptosis operation, ectropion xanthelesma surgery, benign lid lesions surgeries (epidermal nevi, actinic keratosis, fibroepithelial papilloma) have been done with CO2 laser (ultrapulse 5000, Coherent Medical Inc.) Ablation mode (250-500 mJ, 600-900 msn, 10600 nm) was used for benign ocular lesions and resurfacing and incision mode (continous wawe, 0.2 mm spot diameter 5-6 Watt power) was used for cosmetic blepharoplasties and lid malpositions. Results: Ablation technique gave satisfactory results for the treatment of flat periocular lesions and resurfacing. Postoperative prolonged erythema was the most important complication in this group. Hyperpigmentation was seen in 3 patients, and bacterial infection was seen in 1 patient in this group as well. For incision (cutting) mode surgies, less haemorrhage than in conventional surgery was marked, although, late wound healing was noticed in comparison with conventional surgery. Conclusion: CO2 laser surgery is time-saving surgery for the surgeon due to the simultaneous cutting and coagulation capacity.
