Abstract

Aim: To evaluate the clinical features of atypical anterior optic neuropathy of ocular toxoplasmosis and its diagnosis and treatment options Material and Method: Six atypical anterior optic neuropathy form of the 57 patients who have been diagnosed by active ocular toxoplasmosis (first attack, and/or recurrence) between 1996-2007 in Uvea Department of Ministiry of Health Istanbul Education and Research Hospital Eye Clinic were included the study. The average age of the patients was 27 and F/M ratio was 3/3. Visual acuity and intra ocular pressure values of the patients were examined, and their biomicroscopic examinations were done. For differential diagnosis chest x-ray, magnetic resonance imaging of the brain and orbit were taken. Serological tests of toxoplasma, HSV, CMV, SY, tuberculosis, rubella and brucellosis were done. Fundus fluorescein angiographies of the patients were taken. Results: Five of the 6 patients who were diagnosed with atypical anterior optic neuropathy had an old, small, pigmented chorioretinitis scar in the same eye. Toxoplasma IgG antibodies of all patients were positive whereas IgM values were negative. Sulfa drugs and primethamine were given to the patients diagnosed with ocular toxoplasmosis, and after 48 hours, they were treated with systemic corticosteroid. Within the first week, visual acuity increased and the anterior and posterior segment findings started to recover. Conclusions: The differential diagnosis in atypical anterior optic neuropaty cases must be done considering the possibility of atypical involvement of ocular toxoplasmosis.
