Abstract

PURPOSE: To determine the retrobulbar hemodynamic changes of gloucoma patients after trabeculectomy by colour Doppler USG. MATERIAL AND METHOD: 26 eyes of 24 patients were examined preoperatively and postoperatively in two weeks by colour oppler ultrasound prospectively. Systolic maximum velocity end-diastolic velocity and resistance indexes of opthalmic artery central retinal artery, temporal posterior short cilier artery were analyzed. RESULTS: Intraocular pressures measured preoperatively and postoperatively in second week were found statistically significant (p<0,001). The increase in maximum systolic velocity and end-systolic velocity was insignificant and decreasing of resistance index was insignificant in ophthalmic artery (p>0,05). The increase in maximum systolic velocity and end-diastolic velocity and decrease in resistance index of central retinal artery was found statistically significant (p<0.005). The increase in systolic maximum velocity and decrease of resistance index of temporal posterior cliary artery were insignificant (p>0.005) but end-diastolic velocity increase was significant (p<0.005) CONCLUSION: The decrease in intraocular pressure after trabeculectomy increases the ocular blood flow and causes the descent of resistance index of vascular structure. This causes an increase in retinal perfusion.

Özet

AMAÇ: Glokomlu olgularda trabekülektomi ameliyatı sonrası, retrobulber hemodinamik değişikliklerin renkli Doppler görüntüleme yöntemi ile gösterilmesi amaçlandı. GEREÇ VE YÖNTEM: Bu prospektif çalışmada trabekülektomi operasyonuna karar verilen 24 hastanın 26 gözü preoperatif ve postoperatif ikinci haftada renkli Doppler ultrasonografi çekilerek irdelendi. Oftalmik arter, santral retinal arter ve temporal posterior kısa silier arterin sistolik maksimum hızı, diastol sonu hızı ve rezistans indeksleri değerlendirildi. BULGULAR: Preoperatif ve postoperatif ikinci haftada alınan göz içi basınç değerleri arasındaki fark istatistiksel olarak anlamlı bulundu (p 0.05). Santral retinal arterin sistolik maksimum hız ve diastol sonu hızdaki artış ile rezistans indeksinde meydana gelen azalış istatistiksel olarak anlamlı bulundu (p 0.05). Diastol sonu hızda ise istatistiksel olarak anlamlı bir artış izlendi (p<0.05) SONUÇ: Trabekülektomi sonrası göz içi basıncında meydana gelen düşme, oküler kan akım hızında artış oluştururken, vasküler yatağın rezistans indeksinde düşmeye neden olmaktadır.Bu retinal perfüzyonda artış ile sonuçlanmaktadır.
