Abstract

Purpose: To discuss the complications of various surgical tecniques for treatment of congenital cataracts. Materials and Methods: Files of 38 patients (64 eyes) with congenital cataract treated surgically and followed-up in our clinic between March 2000 and June 2006 were examined. Patients were grouped as group1 and 2 according to age at the time of surgery (below and above 2 years of age respectively). In group1, lens aspiration, posterior capsulorhexis and anterior vitrectomy was standart surgical technique (25 eyes of 16 patients). Above 2 years of age (group2) lens aspiration and posterior chamber intraocular lens implantation (IOL) were done in 39 eyes of 22 patients. In 15 of these eyes (group 2a) posterior capsulorhexis and anterior vitrectomy in addition to lens aspitation and intraocular lens implantation were done. In others (group 2b) lens aspiration and IOL implantation were done. Results: Average age at the time of surgery was 45.52±38.56 months (1 month-13 years). Seventeen patients (44.7%) were female, 21 patients (55.3%) were male. Mean follow-up time was 25.18±19.49 (3-71) months. During follow-up, complications were detected in 23 eyes (35.9%). One eye had iris prolapsus (1.56%), 2 had iris capture (3.12%), 3 had early postoperative anterior chamber fibrinoid reaction (4.68%), 3 had pupillary irregularity (4.68%) and 14 had posterior capsule opacification (PCO) (21.84%). PCO was seen 15.35±12.67 (3-48 months) months after initial surgery. Two eyes had PCO in group1 and 12 eyes had PCO in group2. PCO was statistically significantly higher in group 2b compared to group 2a (P<0.001). No complication which could lead to eye loss was seen. Conclusions: Most frequent complication after pediatric cataract surgery is PCO. The incidance of this complication, which can lead to amblyopia, is decreased but not totally eradicated by doing posterior capsulorhexis and anterior vitrectomy. In pediatric age group, primary posterior capsulorhexis and anterior vitrectomy should be included in congenital cataract surgery to decrease the risk of PCO development.
