Abstract

Purpose: To investigate the effects of preexisting glaucoma and pseudoexfoliation (PEX) in eyes which underwent phacoemulsification, in terms of complications, postoperative course, and visual outcome. Material and Methods: A study group of 100 patients were formed among patients in whom phacoemulsification has been performed, with four subgroups each consisting of 25 cases according to preexisting glaucoma and PEX. Results of 81 eyes of 81 patients who attended the follow-up visits regularly were evaluated. Group 1 included 20 cases without glaucoma or PEX, Group 2 included 20 cases with high-tension glaucoma, Group 3 was formed by 16 cases with PEX only, and Group 4 consisted of 25 cases with PEX glaucoma. Results: Mean follow-up time was 18.0±5.2 months. No significant difference was found between groups in terms of intraoperative complications such as posterior capsular rupture or zonular dialysis, however, overall complication rates were apparently lower in Group 1 (5%) and higher in Groups 3 and 4 (25% and 24%, respectively)(p=0.303). Complication rates were higher in eyes in which capsular tension rings were implanted because of dense PEX (p=0.015). Scores of corneal edema and anterior chamber inflammation were significantly higher in eyes with PEX in the early postoperative period. Significant decreases in mean intraocular pressure levels were found at the 12th month visits in all groups except for Group 4. Visual acuity levels were significantly higher in Group 1 in all chosen time intervals. Conclusion: Both preexisting glaucoma and PEX have negative effects on the course of surgery, duration of visual rehabilitation, and outcome in eyes which undergo phacoemulsification
