Abstract

Aim: To assess results for surgical treatment of eyelid malignant tumors after excision and eyelid reconstruction with frozen section control. Material and Method: Records of 38 patients with histological diagnosis of basal cell carcinoma, squamous cell carcinoma, sebaceous gland carcinoma, treated surgical excision and eyelid reconstruction between January 1998 and 2008 were included in this study. Patients were investigated age, gender, follow up period, lesion size, anatomical location, eyelid reconstruction techniques and postoperative results. Results: Age range was 48-82 (mean 67.3±9.4) years. Anatomical location of lesions were lower lid in 20 patients (52.6%), upper eyelid in 8 patients (21%), medial canthal region in 4 patients (10.5%), outer canthal region in 2 patients (5.2% ), lower lid and medial canthal region in 2 patients (5.2%), lower lid and outer canthal region in 2 patients (5.2%). Mean lesion size were 10.7±5.4 (4-26) mm. Patients were followed up 12-36 (mean 18.7±5.6) months. Eyelid reconstruction were made by free skin grafts at 10 patients, ear cartilage for posterior lamella and sliding flap for anterior lamella at 10 patients, primary suture and lateral canthotomy at 5 patients, semicircular flap of Tenzel at 4 patients, ear cartilage and transposition flap from upper eyelid at 3 patients, nasojugal flap at 2 patients, periosteal flap for posterior lamella and skin grafts for anterior lamella at 1 patients, Hughes procedure at 2 patients, Cutler-Beard procedure at 1 patients. 35 (92.1%) patients with eyelid reconstruction had been good functional effects and acceptable cosmetic results. Four (10.5%) patients have complications. Conclusion: The principles of management of malignant eyelid lesions are surgical excision. Recurrence rates can be reduced and eyelid reconstruction can be easier because of minimize tissue loss in tumor excision using frozen section control.
