Abstract

Aim: To evaluate the surgical management of patients with consecutive exotropia. Material and Method: The clinical records of 520 patients who underwent surgery for esotropia from 1990 to 2008 were reviewed retrospectively. Twenty patients with consecutive exotropia were included in this study. The clinical characteristics and surgery outcomes of these patients were evaluated. Medial rectus (MR) resection and lateral rectus (LR) recession was performed in 5 patients, MR advancement and resection with LR recession was performed in 7 patients, bilateral LR recession was performed to 5 patients and only one LR recession was performed in 3 patients. Four patients underwent inferior oblique surgery at the same surgery. Result: Mean age was 14.05 years (3-32) and male/female ratio was 8/12. The mean interval between initial surgery and surgery for consecutive exotropia was 58.7 months (range from 3 months to 343 months). Amblyopia was present in 5 patients (%25). Two of all patients (10%) had fusion. Preoperative 18 patients had fusional supression and postoperative this supression was disappeared at 7 of them. V-pattern was present in 2 patients (10%), (DVD) was present in 1 patient (%5) and convergence insufficiency was detected in 4 patients (% 20). The mean preoperative exodeviation was 33.8±11.7 prism diopters (PD) and postoperative deviation was found 6.5±6.0 PD. Postoperative 7 patients (35%) had exodeviation above 15 PD. Four of them underwent third surgery. Treatment success rate was found % 65 with one surgery and additional surgeries enhances this ratio to 90%. Conclusion: The true surgical treatment of consecutive exotropia should be adopted with paying attention to different characteristics of each patient. LR resection, with advancement and resection of the previously recessed medial rectus seems to be the most effective procedure for consecutive exotropia.

Özet

Amaç: Konsekütif dışa şaşılık gelişen olgularda uygulanan cerrahi tedavi yaklaşımlarının değerlendirilmesi amaçlanmıştır Gereç ve Yöntem:990-2008 yılları arasında içe şaşılık nedeniyle opere olmuş 520 hasta retrospektif olarak incelendi. Konsekütif dışa şaşılık gelişmiş 20 hasta çalışmaya alındı. Has - taların preoperatif karakteristik özellikleri ve cerrahi sonuçları değerlendirildi.
