Abstract

Objective: To evaluate the effect of preoperative corneal endothelial cell count on postoperative oedema in cataract cases that is applied phacoemülsification. Material and Method: Sixty eyes of 58 cataract patients that is applied phacoemülsification without complication that is implanted intraocular lens and followed up at least three months is included in this study. Endothelial cell count is evaluated by specular microscopy and central corneal thichness is evaluated by pachymeter. We use ‘Corneal Oedema Ephron Scale‘ for grading corneal oedema. According to preoperative endothelial cell count, patients were divided in three groups which consisted of 20 patients. Patients in group I have 1000- 2000/mm2 endothelial cell count, patient in group II have 2000-25000/mm2 endothelial cell count and patients in group III have greater than 2500/mm2 endothelial cell count. Results: Postoperatively at first week endothelial cell loss rate was 16.7%, 11,8% and 24.3 % group I, group II, group III respectively. Postoperatively after one week endothelial cell loss rate difference was not statistically significant in each of three group (p >0.05). In each three group increase in central corneal thickness rate was significantly higher in postoperative first day than preoperative level (p<0.01). Central corneal thickness decreased to preoperative level and remained stable in postoperative third month. Corneal oedema rate in postoperative first day and first week was significantly higher in group I than group II and group III (p<0.05). Corneal oedema rate was not statistically significant between group II and group III postoperatively at first day and first week (p>0.05). At the end of postoperative first week corneal oedema rate decreased at the great portion at the cases. Corneal oedema rate was not statistically significant among three groups postoperatively at first and third month (p>0.05). Conclusion: We believed that the lower endothelial cell count at preoperatively the higher the possibility of approach to physiologic threshold,so the higher corneal oedema rate at postoperative first day.
