Abstract

Purpose: To examine the effects of a single drop of 0.2% brimonidine tartrat (Alphagan®) on the retrobulbar hemodynamics in primary open angle glaucoma patients. Material and Methods: Forty-two eyes of 21 consecutive newly-diagnosed bilateral primary open-angle glaucoma patients were enrolled. Each eye of patients was randomly assigned to receive a single drop of 0.2% brimonidine or placebo. The baseline measurements for the retrobulbar blood flow velocities by color Doppler imaging, intraocular pressure by Goldmann tonometer, blood pressure by cuff, and heart rate by palpation were done. All measurements were repeated 2 and 8 h after dosing. Results: Brimonidine lowered intraocular pressure significantly (p<0.001); with a maximal change of 6.5 mm Hg 2 h after dosing. Brimonidine did not alter the hemodynamics in the ophthalmic, posterior ciliary and central retinal arteries. The systolic (p=0.71), and diastolic (p=0.07) blood pressures and pulse rate (p=0.82) remained unchanged over the 8 h-period. Conclusions: This study suggests that topical brimonidine significantly reduces intraocular pressure. However, it does not have any acute effects on the retrobulbar hemodynamics in primary open-angle glaucoma patients.
