Abstract

To evaluate the clinical findings, therapeutic approaches and prognosis in cases who were diagnosed as sino-orbital fungal infection. Three cases with sino-orbital fungal infection were evaluated retrospectively. There were history of uncontrolled diabetes in two cases and long term systemic corticosteroid treatment in one case. All of the cases were referred to our clinic with painful ophthalmoplegia, proptosis and worsening of the clinical findings despite various antibiotic and antifungal therapies. All cases were submitted to intravenous liposomal Ampothericin B treatment. Additionally, in first case, maxillectomy, ethmoidectomy, orbitectomy, nasal cavity and palate excision, in second case; exanteration and sphenoethmoidectomy, in third case; endoscopic sphenoethmoidectomy and localized Amphotericin B irrigation were performed. In first two cases, biopsy materials showed necrosis, micro-abscess formation and broad, wide-angled branching non-septated hyphae demonstrating mucormycosis. In the third case, Aspergillus Fumigatus was isolated from tissues taken with endoscopic sphenoid sinus biopsy. First two patients died on 15th and 60th days of the treatments, respectively. In the last case clinical and radiological improvement were achieved after the treatment and no recurrence was during the first year of follow-up period. Sino-orbital fungal infections have some clinical difficulties since they can easily masquerade different clinical pathologies. A definite diagnosis is necessary in all cases as the treatment comprised radical surgery and systemic anti-fungal medications. Immunsuppression, systemic deterioration and uncontrolled diabetes usually predispose to sino-orbital fungal infections which have a high mortality rate even with appropriate therapy.
