Abstract

Purpose: To evaluate surgical results and complications of the surgery for treatment of lens dislocations in patients with homocystinuria. Material and Methods: Patients with lens dislocation into the anterior chamber with/without pupillary block glaucoma or severe visual loss due to lens dislocation were operated under general anesthesia. Pars plana lensectomy, anterior vitrectomy and peripheral iridectomy were performed and the outcomes were evaulated prospectively. Results: Eight eyes of 4 patients were included in the study. There were dislocation of the lens into anterior chamber and acute pupillary block glaucoma in 75% of the eyes. Intraocular lens implantation was not performed in any of the surgeries. Mean follow-up time was 9.2±4.8 months (2-13 months). No peroperative complication was encountered. Preoperative visual acuity of the patients ranged between hand motions to finger counting, and increased to a range between hand motions and 0.7 postoperatively. We observed secondary glaucoma with severe anterior staphyloma in one eye of a patient 2 months after surgery. Conclusion: Early surgical treatment of lens dislocations should be considered in patients with homocystinuria before the development of pupillary block glaucoma, especially in those where the lens is dislocated into the anterior chamber or causing severe visual loss or ambliyopia. Pars plana vitrectomy and lensectomy is a safe and effective method.
