Abstract

Purpose: To investigate foldable intraocular lens (IOL) movement in anteroposterior axis, measured as a change in anterior chamber depth (ACD) caused by accommodation in pseudophakic eyes Material-Methods: In this study, fortytwo eyes of 42 patients who were scheduled for phacoemulsification and foldable IOL implantation were studied. Before and two months after surgery, anterior chamber depth (ACD) was determined by an Orbtek Orbscan Corneal Topography during physiologic near-point accommodation and two-hour after cyclopentolate 1% was instilled. Preoperative and postoperative ACD measurements were compared as before and after cyclopentolate instillation. Results: The mean age of the patients (20 female, 22 male) was 60.3±10.9 years. Preoperative ACD was 3.61±0.35 mm during physiologic near-point accommodation, 3.67±0.42 mm after cyclopentolate instillation. Postoperative ACD was 4.49±0.38 mm during physiologic near-point accommodation, 4.58±0.27 mm after cyclopentolate instillation. The postoperative mean ACD was significantly higher than preoperative mean ACD. There was no significant difference in IOL movement between near-point accommodation and cyclopentolate instillation. Conclusion: With accommodative effort, ciliary muscle contracts and expanded capsular bag might allow the IOL to move forward. The anterior movement of IOLs in pseudophakic patients may provide good near vision without additional correction. But in this study, there was no significant difference in IOL movement in anteroposterior axis, caused by accommodation in pseudophakic eyes. In the future, foldable IOLs may be designed to allow true pseudophakic accommodation.

Sonuç: Akomodasyon etkisiyle siliyer kas kasılır ve kapsüler cebin genişlemesi GİL’in öne doğru hareketine izin verebilir. Psödofak hastalarda GİL’in öne doğru hareketi ilave düzeltmeye gerek kalmadan iyi bir yakın görme sağlayabilir. Fakat çalışmamızda psödofak gözlerde akomodasyonla GİL’in ön arka eksende belirgin bir hareketi gözlenmedi. Gelecekte, gerçek psödofakik akomodasyona izin verecek katlanabilir GİL tasarımları yapılabilir.
