Abstract

Purpose: To evaluate the results of Nd:YAG laser goniopuncture in viscocanalostomy and to investigate the rate of penetration in non-penetrating surgery. Materials and Method: Results of viscocanalostomy and incidence of goniopuncture were retrospectively investigated in 33 eyes of 33 consecutive patients who were followed-up regularly for at least 2 years. Surgical success was defined as an intraocular pressure (IOP) equal to or lower than 18 mmHg with an at least 30% decrease compared to the last preoperative measurement. Results: General success rates after a mean follow-up of 33.5±11.3 months were 79% and 46%, with and without medications, respectively. Mean IOP levels, which were 37.7±9.9 at the diagnosis and 27.7±11.3 at the last preoperative measurement, decreased to 12.7±4.8 mmHg at the last follow-up visit (p<0.001 and p<0.001, respectively). Twelve eyes (36%) underwent laser goniopuncture to relieve the elevation of IOP during controls. Mean IOP, which was 26.0±4.6 before goniopuncture, decreased to 11.5±4.0 mmHg immediately after the procedure (p=0.002). Early goniopunctures (before month 3) were performed in 6 eyes (mean interval 1.6±1.1 months), and late goniopunctures (after month 10) were performed in the remaining 6 (mean interval 12.8±4.2 months). Success rates of laser-performed eyes at the last visit were 75% and 33%, with and without medications, respectively (p=0.568), while no significant difference was found in terms of IOP levels and reduction (p=0.618). Iris incarcerations were observed in 2 eyes following goniopunctures. No goniopuncture was needed in the 6 eyes with intraoperative macro/microperforations (p=0.049). IOP levels at the first postoperative visit were significantly higher in eyes that underwent goniopuncture (11.8±8.5 vs. 7.1±3.7, p=0.032). Conclusion: Laser goniopuncture is indicated in a significant proportion of patients following viscocanalostomy and substantially improves the outcome of surgery. Reported intraoperative and postoperative penetration rates in non-penetrating glaucoma surgery are noteworthy.
