Abstract

Purpose: To determine accuracy of clinical diagnosis of eyelid malign tumors. Materials and Methods: The medical records of all patients with malign eyelid tumor who underwent an excisonal surgery between January 1997 and February 2006 were reviewed. Recorded data included patient's age, sex, preoperative clinical diagnosis and postoperative permanent section-histological diagnosis. Patients not consistent with the study criteria (failure of record, presence of a recurrent tumour or invasion to the orbit, presence of a diagnostic biopsy) were excluded. Preoperative diagnosis noted in the patient's file notes preoperatively were matched with the results of permanent section-histological examination. The rate of patients whose the clinical and histological diagnosis were in concordance was calculated. Results: A total of 85 patients ( 53 men and 32 women; age range: 32 - 89 years; mean age: 62 years) who were consistent with the study criteria was determined. Each patient had only one malignant eyelid tumour. Histopatological diagnosis was basal cell carcinoma (BCC) in 76 patients, squamous cell carcinoma (SCC) in 7 patients, and sebaceous gland carcinoma (SGC) and malignant melanoma (MM) each in one patient. Preoperative clinical diagnosis was correct in 72 of patients with BCC (95%), in 3 of patients with SCC (43%) and in each of patients with SGC and MM. The overall diagnostic accuracy rate was 91% (n=77/85). Conclusion: The accuracy rate of clinical diagnosis is high in periocular BCC, but relatively low in SCC. Clinical diagnosis in the typical BCCs may be enough to plan the surgical treatment.
