A 9 years old boy with a mass of eight mm diameter which was suspected from and followed with the diagnosis of foreign body granuloma has undergone excisional biopsy. His preoperative appearance, and the results of gross and histopathologic examinations presented. Intraoperatively, the mass considered as pilomatrixoma due to its hard and calcified capsule. Histopathologic examination supported this diagnosis with typical basophilic and ghost cells. Sağ alt göz kapağında 8 mm çaplı bir kitle ile başvuran ve yabancı cisim granülomu ön tanısı ile 1 ay izlendikten sonra kitlesi eksize edilen 9 yaşında erkek hasta preoperatif klinik özellikleri, kitlenin makroskobik ve mikroskobik görünümleri ile sunuldu. Cerrahi esnasındaki sert kapsüllü görünüm pilomatriksomayı düşündürdü. Histopatolojik inceleme, karakteristik bazofilik ve hayalet hücrelerle aynı tanıyı destekler nitelikteydi. Bu olgunun sunulmasının nedeni, oftalmologların diğer kapak kitlelerine göre oldukça nadir gördüğü bir tümör olan pilomatriksomanın hatırlatılması ve epidermoid kist, dermoid kist ve yabancı cisim granülomunun ayırıcı tanısında düşünülmesi gerektiğinin irdelenmesidir.
