Abstract

Aim: Autonomic neuropathy is a common complication in diabetic patients. In recent years apraclonidine 0.5% testing was found useful in the diagnosis of pupillary sympathetic denervation (PSD) and Horner syndrome. We aimed to investigate the prevalence of PSD in diabetes mellitus (DM) by using this technique. Material and Methods: Apraclonidine 0.5% was applied to the eyes of 66 patients with DM and 64 age and sex matched control subjects. The degree of diabetic retinopathy and duration of DM were recorded for each patient. Pupil diameters were measured before and 1 hour after the instillation of the drop. Results: An average of 1.14 mm dilatation in DM group and 0.06 mm myosis in control group was observed after the instillation of apraclonidine, as the difference was statistically significant. At least 1 mm mydriasis was observed in 62% of DM patients. Statistically the degree of pupillary dilatation was highly correlated with duration of DM (r =0.64, p<0.01) and severity of diabetic retinopathy (r =0.83, p<0.01). Conclusion: The severity of PSD which is common in DM was correlated with the duration of DM and presence of diabetic retinopathy. Due to Apraclonidine testing is a non invasive test and easy to perform, it is an eligible method to identify PSD in diabetic patients.
