Abstract

Aim: To examine the effect of thyroid surgery on the course of orbitopathy in patients with thyroid orbitopathy. Material and Method: Fourteen thyroid orbitopathy patients who had been in follow-up in our clinic between December 2003 and January 2009 and underwent thyroid surgery were investigated. Patients were divided into two groups prospectively as ‘steroid users’ and ‘nonusers’. Best corrected visual acuity measurement, biomicroscopic examination, fundus examination, intraocular pressure, proptosis and lid retraction measurement were done in patients of each group. Life quality scoring and Clinical Activity Scoring (CAS) were assessed. All patients underwent orbital computerized tomography (CT) imaging to evaluate the extraocular muscles and optic nerve anatomy. Results: Of 14 patients included in study 12 patients had near total and 2 patients had total thyroidectomy. Oral steroid therapy was started in 8 patients within the first postoperative week. Both steroid user and non-user groups had similar age and gender distribution (p=0.317 and 0.538, respectively). There was no statistical difference between preoperative and postoperative values of both groups for visual acuity, mean exophthalmometry measurement, mean lid retraction measurement and clinic activity score (p>0.05). According to life quality score 1 case was better and 3 cases were worse in steroid user group and 1 patient was better and 2 patients were worse in the non-user group at the end of postoperative 6 month. Conclusion: Near total or total thyroidectomy did not have a significant effect on the course of thyroid orbitopathy in this study. Furthermore, use of oral steroids in the postoperative period did not influence the course of thyroid orbitopathy. Small number of patients and the short follow-up of time are the limitations of this study, therefore it should be considered as a pilot study.
