Abstract

Aim: To evaluate the late results of photodynamic therapy with verteporfin in eyes with subfoveal choroidal neovascularization secondary to age related macular degeneration. Material and Method: Eighteen eyes of 15 patients with subfoveal choroidal neovascularization secondary to age related macular degeneration had photodynamic therapy with verteporfin between November 2005 and December 2006. All patients had full ophthalmologic examination prior to and at 3 month intervals after the treatment, and fundus fluorescein angiography was performed when necessary. Retreatment was performed in the presence of an active lesion. Pre and post-treatment visual acuities and presence of an active lesion were evaluated. Results: Mean age of the patients was 70.1±10.0 years. Minimally classical lesion was found in 5 (27.8%) eyes and predominantly classical lesion was found in 13 (72.2%) eyes. Mean follow up period was 27.7 months and mean number of treatment was 1.7. Two (11.1%) eyes had severe visual loss, 4 (22.2%) eyes had moderate visual loss, 11 (61.1%)eyes had stable visual acuity, and 1 (5.5%) eye had an improvement in visual acuity during the follow up. Conclusion: Despite the requirement of retreatments, photodynamic therapy with verteporfin preserves visual acuity in long term follow up in eyes with subfoveal choroidal neovascularization.

Özet

Amaç: Yaşa bağlı maküla dejenerasyonuna bağlı subfoveal koroid neovaskülarizasyon olgularında verteporfin ile fotodinamik tedavi geç dönem sonuçlarını değerlendirmek. Gereç ve Yöntem: Kasım 2005 ve Aralık 2006 tarihleri arasında yaşa bağlı maküla dejenerasyonuna bağlı subfoveal koroid neovaskülarizasyonu (KNVM) gelişen15 hastanın(11 erkek- 4 kadın) 18 gözüne verteporfin ile fotodinamik tedavi (FDT) uygulandı. Hastalara tedavi öncesi ve sonrası 3’er aylık aralarla tam oftalmolojik muayene ve gerekli görülen olgularda fundus flöresein anjiyografi incelemesi yapıldı. Aktif lezyon varlığında tedavi tekrar uygulandı. Hastalar tedavi öncesi ve sonrası görme keskinlikleri ve takip sonunda aktif lezyon varlığı açısından değerlendirildi. Bulgular: Hastaların ortalama yaşı 70,1±10,0 yıldı. Lezyonların 5’i (%27,8) minimal klasik iken 13’ü (%72,2 ) baskın klasik tipte idi. Ortalama takip süresi 27,7 ay ve ortalama tedavi sayısı 1,7 idi. Takipler süresince 2 (%11,1) gözde ağır görme kaybı, 4 (%22,2) gözde orta şiddette görme kaybı, 11 (%61,1) gözde görme keskinliğinde stabilizasyon ve 1 (%5,5) gözde görmede artış tespit edildi. Sonuç: Verteporfin ile fotodinamik tedavi subfoveal koroid neovaskülarizasyon olgularında tekrarlayan tedaviler gerektirmesine rağmen uzun dönem takiplerde görme keskinliğinin korunmasını sağlamaktadır.
