Abstract

Objective: To evaluate the visual and clinical outcomes of AlphaCor artificial cornea implantation. Material and Method: Eight eyes of 8 cases who underwent Alphacor artificial cornea implantation for high risk in keratoplasty surgery were included to the study. The additional interventions during and after AlphaCor implantation, complications and visual outcomes were evaluated. Results: Cases were followed-up minimum 12 and maximum 20 months with a mean period of 15.6 ±3.4 months. In 3 cases additional phacoemulsification and foldable intraocular lens implantation surgery was performed. The anterior stromal lamella was covered with Gunderson conjunctival flap in 3 cases who had intraoperative perforation. During the follow-up in 5 cases (62.5%) stromal melting developed and in 3 (60%) of them AlphaCor artificial cornea was removed and penetrating keratoplasty (PK) was performed. Graft decompensation after PK due to the graft rejection had developed in all cases. After second level surgery intraoptic and superficial deposits were seen in 3 patients (37.5%). Preoperatively visual acuity was hand motion. At the final examination visual acuity was between hand motion and 0.1 and functional vision (0.05 or better) was achieved in only 2 cases (25%). Conclusion: AlphaCor artificial cornea implantation is an alternative treatment option in high risk eyes that has low chance of donor cornea graft survival. However, AlphaCor was found to have high risk for post implantation stromal melting and implant dislocation. Preservation of bulbus integrity with corneal grafts even in these serious complications seems to be the major advantage of AlphaCor artificial cornea implantation.
