Abstract

Aim: To evaluate the ability of ganglion cell complex (GCC) parameters determined by spectral-domain optical coherence tomography (OCT) to diagnose glaucoma. Material and Method: In this prospective study, 69 eyes with open angle glaucoma and 70 age and sex matched healthy eyes (control group) were assessed. Glaucomatous eyes were fur- ther divided into early glaucoma and advanced glaucoma subgroups. Visual field test was per- formed and OCT (RTVue, Optovue) was used to determine GCC thickness, focal loss volume (FLV), global loss volume (GLV), retinal thickness (RT), and retinal nerve fiber layer (RNFL) thickness in all subjects. Sensitivity and specifity of FLV, GLV, GCC/RT ratio and RNFL meas- urements to diagnose glaucoma were calculated. Results: GCC/RT ratio was significantly different between control group, early glaucoma- tous eyes and advanced glaucomatous eyes (p<0.05). The mean FLV was 0.55±0.89%, 1.34± 1.57% and 7.12±5.60% in the control group, early glaucoma group and advanced glaucoma group, respectively, whereas the mean GLV was 3.57±4.49%, 5.36±4.01% and 16.47±12.89%; and the mean RNFL thickness was 107.71±17.10μm, 102.33±10.15μm and 82.87±19.74μm, respectively (all; p<0.05). Of the analyzed parameters, GLV had the highest specifity, and GCC/RT ratio had the highest sensitivity. Conclusions: GCC parameters and GCC/RT ratio determined by spectral-domain OCT showed high sensitivity and specifity in detecting glaucoma. While diagnosing glaucoma, in- terpreting these parameters together will guide the clinician for early diagnosis and treatment.
