Abstract

Aim: To investigate the presence of diabetic retinopathy (DR) and the stage of diagnosed retinopathy at the first aplication of patients to the ophthalmolgy outpatient clinic. Material and Method: The type 1 and type 2 diabetic patients that aplied to Gaziantep Dr. Ersin Arslan State Hospital ophthalmology clinic for the first time between June 2011 and June 2012 were included in the study. Fundus fluorescein angiography findings and records of 226 diabetic patients were analyzed. Ninty-six diabetic patients who were consulted for possible DR from internal medicine clinic was classified as group 1 (Internal medicine group), 130 di- abetic patients who have admitted directly to ophthalmology clinic by chance or any eye com- plaint was classified as group 2 (Eye outpatient clinic group). Results: The presence of DR was detected in 54 (56.2%) of internal medicine group pa- tients. The presence of DR was detected in 117 (90%) of eye outpatient clinic group. In inter- nal medicine group, nonproliferative diabetic retinopathy (NPDR) was diagnosed in 52 (96.3%) of and proliferative diabetic retinopathy (PDR) was diagnosed in 2 (3.7%) of patients with diabetic retinopathy. In outpatient eye clinic group, PDR was diagnosed in 44 (37.6%) and NPDR was diagnosed in 73 (62.4%) of the patients. Macular edema was detected in 28 (29.2%) of patients in group 1 and in 68 (70.8%) of patients of patients in group 2. Conclusion: In the limited area of study hospital, diabetic patients have applied quite late for the first eye examination and DR is detected at advanced stages. Thus, ophthalmology con- sultations from internal medicine departments are necessary for early diagnosis of DR.

Özet

Amaç: Hastanemiz göz hastalıkları polikliniğine ilk defa başvuran diyabetik hastaların di- yabetik retinopati (DR) varlığının ve retinopatinin ilk göz muayenesinde hangi evrede teşhis edildiğinin araştırılması. Gereç ve Yöntem: Çalışmamıza, Haziran 2011-Haziaran 2012 tarihleri arasında Gazian- tep Dr. Ersin Arslan Devlet Hastanesi göz kliniğine ilk defa göz muayenesi olmak için başvu- ran tip 1 ve 2 diyabetik hastalar dahil edildi. İki yüz yirmi altı diyabetik hastanın muayene ka- yıtları ve fundus flöresein anjiyografi bulguları incelendi. Dahiliye kliniklerinden olası DR tes- piti için konsülte edilen 96 diyabetik hasta grup 1 (Dahiliye grubu), herhangi bir göz şikayeti nedeniyle ya da tesadüfen göz polikliniğine başvuran 130 diyabetik hasta grup 2 (Göz polikli- nik grubu) olarak sınıflandırıldı. Bulgular: Dahiliye grubu hastalarının 54’ünde (%56,2) DR varlığı saptandı. Göz polikli- nik grubu hastalarının, 117’sinde (%90) DR varlığı saptandı. Dahiliye grubunda DR tespit edi- len hastaların 52’si (%96,3) non-proliferatif diyabetik retinopati (NPDR), 2’si (%3,7) prolife- ratif diyabetik retinopati (PDR) olarak teşhis edildi.
