Abstract

Aim: Effect of phacoemulsification surgery on measurment of retinal nerve fiber thickness in primary open angle glaucoma patients. Material and Method: Twentyone eyes of twentyone patients who were followed in glaucoma department and underwent cataract surgery between April 2012 and October 2012 were included in this study Intraocular pressure measurement (IOP), biomicroscopic and fundus examinations, average cup and disc ratio (C/D), central corneal thickness were investigated retrospectively. Retinal nerve fiber layer (RNFL) thickness, superior and inferior hemispheric values, and signal strenght intensity (SSI) values measured with spectral domain OCT preoperatively and postoperative 1 month were evaluated. Results: Of the patients included to the study 11 (52.3%) were male and 10 ( 47.7%) were female and mean age of patients was 68.12±9.22. Mean IOP was 17.18±3.54 mmHg preoperatively and 12.76±2.17 mmHg postoperatively (p<0.01). Mean RNFL was 81,41±17.52 μm preoperatively and 87.35±16.61μm postoperatively (p<0.01). Mean SSI value was 38.31±9.33 preoperatively and 52.13±7.16 postoperatively (p<0.01). Conclusion: Media opacities may affect the image quality of OCT due to low SSI. Also, in glaucoma patients with cataracts, measurement of RNFL is affected and after phacoemulsification surgery RNFL thickness may be obtained as higher values.

Özet

Amaç: Primer açık açılı glokom hastalarında fakoemülsifikasyon cerrahisinin retina sinir lifi kalınlık ölçümlerine olan etkisinin incelenmesi. Gereç ve Yöntem: Bu çalışmaya Nisan 2012-Ekim 2012 tarihleri arasında glokom biriminde takip edilen ve katarakt cerrahisi uygulanan 21 hastanın 21 gözü dahil edilmiştir. Hastaların ameliyat öncesi ve sonrasında, göz içi basınç (GİB) ölçümü, biyomikroskopi ve fundus muayeneleri, ortalama çukurluk/disk (C/D) oranı değerleri ve merkezi kornea kalınlıkları geriye dönük olarak incelendi. Hastaların ameliyat öncesi ve ameliyat sonrası 1. ayda Spektral Domain Optik Koherans Tomografi (OKT) ile yapılan ortalama retina sinir lifi tabakası kalınlık ölçümü (RSLT) değeri, üst ve alt hemisfer değerleri ve sinyal gücü göstergesi (SSI) değerleri incelendi. Bulgular: Çalışmaya dahil edilen olguların 11’i erkek 10’u kadın ve yaş ortalamaları 68,12±9,22 idi. Ameliyat öncesi ortalama GİB 17,18±3,54 mmHg olup, ameliyat sonrası ortalama GİB 12,76±2,17mmHg’dir (p<0.01). Ameliyat öncesi RSLT ortalaması 81,41±17,52μm olup, ameliyat sonrası RSLT ortalaması 87,35±16,61 μm’dir (p<0,01). Ameliyat öncesi SSI ortalaması 38,31±9,33 olup ameliyat sonrası SSI ortalaması 52,13±7,16’dır (p<0,01). Tüm parametrelerde ameliyat sonrası dönemde istatistiksel olarak anlamlı değişiklik saptanmıştır. Sonuç: Ortam opasiteleri, düşük sinyal yoğunluğuna bağlı olarak OKT cihazının görüntü kalitesini etkileyebilmektedir. Kataraktı mevcut glokom hastalarında ise RSLT ölçümü etkilenmektedir ve fakoemülsifikasyon cerrahisi sonrasında retina sinir lifi kalınlığı ölçümünde daha yüksek değerler elde edilebilmektedir.
