Abstract

Objective: To evaluate the effectiveness of endoscopic transcanalicular laser dacryocystorhinostomy and bicanalicular silicon tube intubation in nasolacrimal duct obstruction revision operation. Material and Method: Nasolacrimal duct obstruction revision operations by endoscopic transcanalicular laser dacryocystorhinostomy with bicanalicular silicon tube intubation were applied to patients without canalicular blockage. Results were retrospectively reviewed. All surgeries were performed by using a multidiode laser. Nasolacrimal duct patency was controlled by lacrimal drainage system irrigation. The anatomic "success" criteria were defined as nasolacrimal duct patency. Results: In this study, 11eyes of 11 patients [Female / Male: 5/6; mean age: 48.36+8.73 years (37-63)] were included. The mean duration time of silicone tube intubation was 2.63±0.64 months (2-4), mean follow up time was 11.36±4.84 months (5-18). The 'success' rates were; 90, 9 % at postoperative 1st week control; 81.8% at postoperative 1st month control; 72.7% at postoperative 2nd months control, 63.6% at postoperative 3th months control. The "success" rate was 54.5% at postoperative the last control after silicone tube removal. The "success" rates of revision endoscopic transcanalicular laser dacryocystorhinostomy and bicanalicular silicon tube intubation were 57.1% in patients having previous external dacryocystorhinostomy operation after 8.85±5.22 months follow-up and 50% in patients having previous laser operation after 9.0±4.30 months follow-up after silicone tube removal. There was not a significant difference between the success rate of revision surgery of patients having external dacryocystorhinostomy and laser surgery. Conclusion: In the revision of nasolacrimal duct obstruction endoscopic transcanalicular laser dacryocystorhinostomy with bicanalicular silicon tube intubation is an effective method in short term but its effect decreases in time. There was not a significant difference in success rate of revision surgery preceeded by external dacryocystorhinostomy or endoscopic transcanalicular laser dacryocystorhinostomy.
