Abstract

A 77-year-old male patient presented with pain, redness and visual loss in the right eye. His best-corrected visual acuity was counting fingers at 2 meters and intraocular pressure was 25 mmHg with medical treatment in the right eye. Slit-lamp examination of the right eye revealed conjunctival hyperemia and bullous keratopathy. There was also a three piece posterior chamber intraocular lens which was implanted in the anterior chamber and was in touch with the corneal endothelium. Intraocular lens was divided into two parts in the anterior chamber and explanted via 3.5 mm clear corneal incision. Patient’s symptoms and signs were markedly improved after the operation. Intraocular pressure was 18 mmHg without any medical treatment after 3 months of lens explantation. Intraocular lens implantation should not be performed in every case at the same time in the absence of adequate capsular support for posterior chamber intraocular lens implantation during cataract surgery. In cases which implantation of an anterior chamber intraocular lens is considered, only special anterior chamber designed intraocular lens’s should be utilized. A posterior chamber designed IOL should never be implanted in to the anterior chamber.

Katarakt cerrahisi sırasında kapsül desteği kaybolan olgularda, her olguya aynı seansta göz içi lens implantasyonu yapılmamalıdır. Ön kamara lens implantasyonu düşünülen olgularda ön kamara için tasarlanmış lensler kullanılmalı, arka kamara lensleri ön kamaraya implante edilmemelidir
