Abstract

The aim of this study; the aim of this study is to investigate the relationship between fatigue and sleep quality of COPD patients. The sample of the research; Between July and December 2018, 209 patients with COPD who were treated in the Chest Diseases Clinic of a public hospital in Balıkesir and accepted to participate in the study were included in the study. Data were collected by using Individual Identification Form, COPD and Asthma Fatigue Scale (RCT) and Richard Campbell Sleep Questionnaire (RCUA). Ethics committee permission was obtained for conducting the study and necessary permissions were obtained from the institution and participants. A statistical program was used for analysis. The mean age of the patients included in the study was 65.96 ± 8.52 (48-90), 55% of whom were 65 years and older and 62.2% of them were male. The mean COPD year of the patients was 11.32 (1-45) and 60.3% of the patients were diagnosed between 1-10 years. While 90.4% of the subjects included additional disease other than COPD, the most common disease was Hypertension (63.6%) followed by Diabetes Mellitus (35.9%). The mean fatigue score of the patients included in the study was 68.11 ± 12.12, and the mean sleep quality score was 61.71±12.44. There was a negatively weak correlation between fatigue and sleep quality of COPD patients (r = -0.361; p <0.001). It was found that sleep quality decreased as the fatigue level of COPD patients increased. It is thought that assessment and management of fatigue in the care of individuals with COPD will improve sleep quality by controlling disease risks.

Özet

Bu çalışmanın amacı; KOAH’lı bireylerin yorgunluk düzeyi ile uyku kalitesi arasındaki ilişkiyi incelemektir. Araştırmanın örneklemini; Temmuz – Aralık 2018 tarihleri arasında Balıkesir’de bulunan bir devlet hastanesinin Göğüs Hastalıkları kliniğinde tedavi gören ve örnekleme dâhil edilme kriterlerine uyarak çalışmaya katılmayı kabul eden 209 KOAH'lı hastalar oluşturmuştur. Veriler, Birey Tanılama Formu, KOAH ve Astım Yorgunluk Ölçeği (KAYÖ) ve Richard Campbell Uyku Anketi (RCUA) ölçekleri kullanılarak yüz yüze görüşme tekniği ile toplanmıştır. Araştırmanın yürütülmesi için etik kurul izni, araştırmanın yapılacağı kurumdan ve katılımcılardan gerekli izinler alınmıştır. Analizler için bir istatistik programı kullanılmıştır. Çalışma kapsamına alınan hastaların yaş ortalaması 65,96±8,52 (48-90) olup %55’i 65 yaş ve üzeri, %62,2’si erkek hastalardan oluşmaktadır. Bireylerin KOAH yılı ortalaması 11,32 (1-45) idi ve %60,3’ünün 1-10 yıl arası tanı alan hastalardan oluştuğu saptandı. Örneklemi kapsayan bireylerin %90,4’ü KOAH dışında ek hastalığı bulunduğunu ifade ederken en çok sahip olunan hastalık birinci sırada Hipertansiyon (%63,6) iken takiben ikinci sırada Diabetes Mellitus (%35,9) idi. Araştırma kapsamına alınan hastaların yorgunluk puan ortalaması 68,11±12,12, uyku kalitesi puan ortalaması ise 61,71±12,44 idi. Çalışmaya dahil edilen KOAH’lı bireylerin yorgunluk düzeyi ile uyku kalitesi düzeyi arasında negatif yönde zayıf düzeyde bir ilişki saptanmıştır (r= -0,361; p<0,001). Araştırma kapsamına alınan KOAH'lı bireylerin yorgunluk düzeyinin arttıkça uyku kalitesinin azaldığı saptanmıştır. KOAH'lı bireylerin bakımında yorgunluğun değerlendirilmesi ve yönetilmesi, hastalık risklerinin kontrol altına alınarak uyku kalitesinin artmasını sağlayacağı düşünülmektedir.
