Abstract

Splenic injury is still a problem during procedures involving the esophageal hiatus, stomach, kidney and the aorta. During the seven year period from September 1994 through November 2001, data sheets have been retrospectively complied on 51 patients undergoing splenectomy at the Department of General Surgery in Pamukkale University. In patients with iatrogenic splenic injury, data were collected concerning age, sex, diagnosis, operative or postoperative blood transfusion and postoperative complications. Eight (5.6 %) of 51 splenectomies performed during this period resulted from iatrogenic injury to the spleen. Traction on peritoneal omental attachments and adhesions was the mechanism of injury in seven patients and avulsion of a small portion of splenic hilum was the mechanism in a single patient. The maneuvers to avoid traction upon the spleen and its attachments can lessen the frequency of this complication. Immediate identification and management by either splenic repair or splenectomy should result in a negligible increase in potential complications.
