Abstract

We report a case of isolated uncalcified hepatic macronodular tuberculoma causing biliary obstruction, which was misdiagnosed as primary hepatic cancer and the correct diagnosis was made pathologically. An 8-year-old boy was admitted to our institution with a one-year histo-ry of hepatomegaly, jaundice, anorexia, and weight loss. The chest x-ray was entirely normal with no history or symptoms/signs of tuberculosis. The abdominal ultrasound and CT scan revealed an infiltrative tumour mass lesion in the liver. Preoperative needle biopsy was nonspecific. The patient underwent surgery with a preliminary diagnosis of liver tumor. Intraoperatively, a solid, hard, and vascularised mass 6 cm in diameter with no capsule formation, located at the medial segment of the left lobe of the liver was noted. A total resection of the tumour including gall bladder was performed. Postoperative pathological evaluation revealed tuberculoid infiltration of the liver. We found no other infiltration site of the disease and no symptoms and complication occured during the 3.S year follow-up. In this report, differential diagnosis, pathogenesis, and therapy of tuberculoid infiltration of the liver are discussed.
