Abstract

Undifferentiated (embryonal) sarcoma of the liver (USL) is a rare malignant tumor with mesenchymal origin. Bronchobiliary fistula is reported as an extremely rare complication in patients with liver tumor. The case presented, is the first report of such a complication in USL. Case: The difficulty encountered in radiological differential diagnosis, resulted in delayed diagnosis of USL of the right liver lobe in a 12 year-old boy. The initial diagnosis was liver mass mimicking postraumatic hematoma. The tumor originated from the 5th and 6th segments of the right liver lobe, invading the 4th segment and anterolateral aspect of the diaphragm. An extended right hepatectomy and total resection of the mass was performed. The family refused further treatment immediately after surgery. Chemotheraphy was initiated three months after when the patient was readmitted with enlarged tumor. MR investigation revealed diaphragmatic invasion by the enlarged tumor, the left liver lobe was tumor free. Six months after the operation, the presence of fever, cough and bilious sputum suggested a bronchobiliary fistula, which was confirmed by scintigraphy. The patient underwent a fistula division. The patient died from a febrile neutropenic attack 11 months after initial diagnosis. Conclusion: Recent experiences have been reported on long-term survivors with combined therapy. In the presented case, the delay in chemotheraphy and diffuse diaphragmatic invasion is thought to be the reason of a late bronchobiliary fistula.
