Abstract

Objective: The aim of this study was to evaluate the general properties, surgical techniques and complications of laparoscopic splenectomy in surgical disorders of the spleen including our clinical experience. Materials and Method: Sixteen patients who underwent laparoscopic splenectomy in SSK Ankara Training Hospital 3rd General Surgery Clinic between January 2000 and December 2004 were examined retrospectively. Results: Nine female and seven male, total sixteen patients were included in this study. The mean age was 36.3 (range 26-54). The most frequent group of patients who underwent laparoscopic splenectomy were young people with immune thrombocytopenic purpura refractory to medical treatment with normal or near normal spleen size. The mean operation time was 98 minutes (range 56-180). There were no patients requiring conversion to open surgery during the laparoscopic period but one of the patients was reoperated because of bleeding on same day of the operation. Revision rate was 6.25%. Conclusion: Laparoscopic splenectomy is a reliable and effective procedure in patients who have minimal morbidity criterias and normal or near normal spleen size with benign haematologic disorders refractory to medical treatment.

Özet

Amaç: Bu çalışma splenektomi gerektiren dalak hastalıklarında laparoskopik yaklaşımın genel özelliklerini, cerrahi teknik ve komplikasyonlarını klinik deneyimimizi de ekleyerek irdelemektir. Gereç ve Yöntem: Ocak 2000 ve Aralık 2004 tarihleri arasında SSK Ankara Eğitim Hastanesi 3. Genel Cerrahi Kliniği'nde laparoskopik splenektomi yapılan 16 hastanın bilgileri retrospektif olarak incelendi. Bulgular: Bu çalışmaya 9 kadın, 7 erkek toplam 16 olgu alındı. Yaş ortalaması 36.3 (min 26-maks 54) idi. Laparoskopik splenektomi yapılan olguların en sık normal ya da normale yakın dalak boyutlarına sahip, medikal tedaviye dirençli genç immün trombositopenik purpura hastaları oldukları görüldü. Operasyon zamanı ortalama 98 dk (min 56-maks 180) idi. Konversiyon yapılan olgu olmadı. Bir olgu kanama nedeniyle aynı gün reopere edildi. Revizyon oranı %6.25 idi. Sonuç: Laparoskopik splenektomi normal veya normale yakın dalak boyutuna ve minimal morbidite kriterlerine sahip, genç, medikal tedaviye dirençli selim hematolojik patolojilere sahip olgularda güvenilir ve efektif bir prosedürdür.
