Abstract

Objective: The aim of this study was to examine serum thrombomodulin (TM) levels before and after ERCP in patients with cholestasis, with attention to related parameters for the evaluation of post-ERCP activation of endothelium. Material and Methods: 30 patients (11 male, 19 female) underging ERCP in our clinic were included in the study. Patients ages ranged from 22 to 78 years, with an average of 56 13 years. Results: In patients underging ERCP, TM levels were significantly higher after the procedure than before (p 0.05). In patients receving endoscopic sphincterotomy (ES), TM levels levels after ERCP were increased to a statistically significant degree (p 0.05), while in patients not receving ES; levels before and after the procedure were not statistically distinguishable (p 0.05). In patients who had episodes of post- ERCP pancreatitis, post-ERCP serum TM concentrations were significantly higher than before the precedure, while in patients who did not develop pancreatitis, there was no significant difference. In this study, with TM levels being taken in early period of 6 hours after ERCP, high post-ERCP TM concentrations appeared to be predictive of post-ERCP pancreatitis. Conclusion: In order to determine the relation between serum TM concentrations, ERCP and post-ERCP pancreatitis in patients with cholestatic joundice, there is a need for comparison to levels in controlled, prospective and larger studies. There is also a need for further studies of the relation between TM levels in acute pancreatitis and other prognostic parameters.

Özet

Amaç: Bu çalışmada çeşitli nedenlerden ERCP yapılan ve kolestazı olan hastalarda işlem öncesi ve işlem sonrası serum TM düzeylerinin incelenmesi, TM düzeylerinin çeşitli parametrelerle ilişkisinin araştırılması ve ERCP sonrası endotel aktivasyonunun değerlendirilmesi amaçlandı. Gereç ve Yöntem: Kliniğimizde ERCP yapılan 30 hasta çalışma kapsamına alındı. Çalışma grubunu oluşturan 30 hastanın 11'i erkek (%36.7) ve 19'u kadındı (%63.3).
