Abstract

Objective: This study aims to define the relationship between preeclampsia and serum folic acid, vitamin B12 and plasma homocysteine levels. Materials and Methods: This study was a case control study performed by a single center. 20 pregnant females with preeclampsia and 30 healthy pregnant females were included in the study from November 2000 to January 2004. The women were primigravida of 18 and 40 years of age and did not use any drugs other than iron supplements. Serum vitamin B12, folic acid and plasma homocysteine levels were measured in all patients at their third trimester of pregnancy. Results: Both groups were similar when compared by criteria of age, body mass index, nutrition and pregnancy week the samples were withdrawn. Vitamin B12 serum levels in the study and the control group were similar (241.1±111.7 pg/ml in the study group and 236±111.1 pg/ml in the control group, p= 0.879). Folic acid levels in the preeclampsia group were significantly low (15.15±6.7 ng/ml in the control and 11.49±8.96 ng/ml in the study group, p= 0.020). Homocysteine levels were significantly high in the preeclampsia group (8.65±2.05 µmol/L in the study group and 6.19±1.52 µmol/L in the control group, p<0.001). Homocysteine levels above 8.65 µmol/L increased the preeclampsia risk 24 fold when compared with the homocysteine levels below 6.19 µmol/L. Conclusion: While homocysteine levels were high in the study group as compared to the control group, plasma folic acid levels were low. Homocysteine increased in overt preeclampsia cases. We recommend correction of nutritional folic acid deficiency in the antenatal period and determination of homocysteine plasma level in the preeclampsia patients.
