Abstract

AIM: After the recognition of fatty echo pattern of the liver by ultrasound, patients are frequently refereed to gastroenterology clinics: We tried to determine the etiologies of our patients with fatty liver (FL). METHODS: Fourty-nine patients (24 male, 25 female, mean age:49.3±l3.2 years) with FL defined by ultrasound were evaluated. Two patients with HCV infection and 4 patients with alcohol history were excluded. Pregnancy or drug usage were also accepted as exclusion criteria^ Forty-three patients (18 male, 25 female, mean age:46.8±14.4 years) were included into the study. Demographical parameters, body mass indexes (BMI), fasting blood glucose levels, oral glucose tolerance, tests, cholesterol and triglyceride levels were measured. :REŞULTSi,$ix(]3,9%)'patients had previouslydiagnosed diabetes mellitus. During.differential diagnosis ofFL, werecognized diabetes mellitus in 9 (20.9%) patients, and impaired glucose tolerance in 6 (13.9%) patients. Twenty-two (51%) patients were obese (BMI>29). Twelve of obese patients had also diabetes at the same time. Eight patients (18.6%) had hypercholesterolemia, 6 (13.9%) had hypertriglyceridemia. We could not determine the etiology ofFL in 3 (6.9%) patients. All these 3 patients had normal transaminase levels. Only 8 (18.6%) out of all patients had increased enzyme levels; 5 with diabetes+obesity, 2 with hypercholesterolemia+obesity, and 1 with diabetes+ hypercholesterolemia-^obesity. CONCLUSIONS: A number of patients were diagnosed to have diabetes mellitus in a gastroenterology clinic during the evaluation of the etiology of FL. Second conclusion was the determination of the etiology in 93% of the patients with FL diagnosed by ultrasound. This high rate may reveal the reliability of ultrasound in the diagnosis of FL as reported in the literature.
