Abstract

OBJECTIVE: Distinguishing characteristics of perennial (PAR) and seasonal allergic rhinitis (SAR) were evaluated in patients with allergic rhinitis which is a significant health problem in childhood and clinically investigated in two subheadings as PAR and SAR. METHOD: Sixty-two patients, 32 males (52%) and 30 females (48%) followed up at the Pediatric Allergy; Unit of Dicle University Hospital with the diagnosis of allergic rhinitis during the last three years were evaluated retrospectively. RESULTS: Thirty-one of all patients were admitted as PAR and 31 as SAR with a mean age of 7.2±2.2 years ranging between 3 to 15 years. The most frequently seen symptom was nasal obstruction in PAR patients (90%) and rhinorrhea in SAR patients (94%). Age at onset of symptoms was found to be significantly different between PAR and SAR patients (6.2±2.1 and 4.2±2.4 years, respectively). Eosinophilia in nasal smear was over 25% in 24 (39%) patients and serum total IgE levels were over the normal values for the approppriate age in 44 (71 %) of all patients. Twenty-five (40%) patients had familial anamnesis of atopic diseases. Association with sinusitis was observed in 18 (29%) and otitis media in 6 (10%) patients. Among 26 (42%) patients accompanying with bronchial asthma 18 patients was in the PAR and 8 were in the SAR group, while 15 of'22 (%35) patients associated with allergic conjunctivitis were in the SAR and 7 were in the PAR group. Sensitivity to pollens was leading in SAR and to house dust mites in PAR patients. CONCLUSION: Diagnosis of allergic rhinitis is established by the appropriate clinical symptoms and the supporting positive allergy tests, eosinophilia in nasal smear and peripheral blood, high serum total IgE levels, familial history of atopy and the presence of other allergic diseases. The most prominent symptom was rhinorhea, the most frequently seen allergen was pollen and associated allergic disease was allergic conjunctivitis in patients with SAR. while nasal obstruction, house dust mite and accompanying bronchial asthma were prominent in patients with PAR.

Özet

AMAÇ: Çocukluk çağında önemli bir sağlık sorunu olan ve klinik görünümü açısından yılboyu süren perennial allerjik rinit (PAR) ile mevsimsel allerjik rinit (MAR) olmak itere iki ali baslıkta incelenen allerjik rinitli hastalarda : PAR ve MAR'ı ayıran özelliklerin değerlendirilmesi amaçlandı. YÖNTEM: Dicle Üniversitesi Ç ocuk Sağlığı ve Hastalıkları Allerji Polikliniği'nde son üç yılda allerjik rinit tanısı ile izlenen 32 'si erkek (%52). 30 'u kız (%48), toplam 62 hasta retrospektif olarak değerlendirildi. BULGULAR: Yaş ortalaması 7.2±2.2 yıl (3-15) olan hastaların 31 'i PAR. 31'i MAR olarak kabul edildi. Tüm olguların 24 'ünde (%39) nazal sürüntüde eozinofil oranı %25 'in üzerinde. 44 'ünde (%71) serum total IgE düzeyi yaşa göre normal değerin üzerinde bulundu. Olguların 25 'inde (%40) ailede atopik hastalık öyküsü vardı. Hastaların 18'inde (%29) sinüzit, 6'sında (%10) otit birlikteliği görüldü. Allerjik rinit ile birlikte bronşta! astım saptanan 26 (%42) hastanın 18'i PAR, 8'i MAR: allerjik konjunktivit saptanan 22 (%35) hastanın 15'i MAR. 7'si PAR olarak tam alan hastalardı. MAR'da polen, PAR'da ev tozu akarlarına duyarlılığın ilk sırada yer aldığı görüldü. SONUÇ: Allerjik rinit tanısı uygun klinik semptomlar ile birlikte tanıyı destekleyen allerji testleri, nazal yaymada eozinofili saptanması, periferik yaymada yüksek eozinofil oranları, yüksek IgE düzeyleri, ailede atopi öyküsü varlığı ve diğer allerjik hastalıkların birlikteliği ile konmaktadır. MAR 'da semptom olarak burun akıntısı, allerjen olarak polenler, birlikte bulunan allerjik hastalık olarak ta allerjik konjunktivit ön planda iken: PAR olgularında hunin tıkanıklığı semptomu, ev tozu akarları allerjisi ve bronşial astım birlikteliği göze çarpmaktadır.
