Abstract

AIM: Hepatic hemangiomas are the most common benign tumors of the liver. In clinical practice, hemangiomas are followed-up for change in size by ultrasound periodically. Recently, there are a few reports claiming that hemangiomas can change in size chronologically. The aim of this study was to disclose the natural behavior of hemangiomas and to evaluate the clinical efficacy of sonographic follow-up; since little is known about the natural course of these tumors. MATERIAL AND METHODS: We performed clinical and sonographic follow-up of 16 hemangiomas in 14 patients. Ultrasound, computed tomography, dynamic computed tomography, Doppler ultrasound, magnetic resonance imaging and magnetic resonance angio were used where needed to make the differential diagnosis of hepatic masses. In two patients, laparoscopy was needed to make the definite diagnosis of hemangioma. The patients were followed-up monthly in first 3 months, then once in 3 months by ultrasound and additional imaging modalities where needed. Underlying liver diseases in these 14 patients included 3 cases with fatty liver, 3 cases with liver cirrhosis, and 2 cases with chronic viral hepatitis. The remaining 6 patients showed no evidence of underlying liver disease. RESULTS: Our follow-up period ranged 12-37 months. The average follow-up period was 19.3±4.5 months. None of the patients presented symptoms which could be attributed to the tumor. None of 16 hemangiomas increased in size during the follow-up period. Only one lesion showed 30% decrease in size. CONCLUSION: Ultrasound seems a safe and effective procedure in the follow-up of hepatic hemangiomas. Thus, if clinically diagnosed hemangioma which tends to increase in size is detected on ultrasound or other imaging modalities, laparoscopy should be preferably performed considering the possibility of hepatocellular carcinoma.
