Abstract

OBJECTIVE: To evaluate the clinical and histopathological factors acting on prognosis and the effect of surgery on survival of stage IV gastric cancer patients. MATERIAL AND METHODS: Stage IV gastric cancer patients diagnosed and treated between 1996-1998 were included in this study. The factors possibly effecting prognosis such as age, sex, tumor location, site of metastasis, macroscopic appearence (Borrman classification) and histologic type (Lauren classification) of tumor, type of surgery and chemotherapy were retrospectively evaluated. Survival analysis and univariate and multivariate analyses of factors effecting survival were performed. RESULTS: 106 patients were included in the study. Patients' mean follow-up time was 14 months (1-40 months) and median survival time was 11 months and overall 3-year survival was 8.8%. The factors effecting survival on univariate analysis were tumor location, type of surgery and chemotherapy. Tumors involving the gastric wall as a whole had a shorter survival. Surgical resection prolonged survival and addition of chemotherapy was found as a factor positively effecting prognosis. Multivariate analysis revealed that only chemotherapy independently prolonged survival. CONCLUSION: When selectively performed, radical surgical resections enabling the removal of both macroscopic and microscopic tumor may prolong survival in stage IV gastric cancer patients with a well known worse prognosis. Adjuvant chemotherapy will further prolong survival.
