Abstract

AIM: The aim of the study was to investigate the sedation, analgesia and recovery profile of the bolus remyfentanyl and midazolam or propophol infusion in cystoscopic procedures. MATERIAL AND METHODS: After necessary monitorisation 0,5 mcg.kg-1 bolus remifentanil was delivered intravenously. Group I received 1 mcg.kg-1.min-1 midazolam and Group II received 50 mcg. kg-1.min-1 propofol infusion for sedation. Heart rate, mean blood pressure and peripheral blood hemoglobin oxygen saturation were noted through out the procedure. The infusion rate of the study medications was titrated according to the level of sedation. Anesthesia and surgery times were also noted. After the procedure, the Aldrete recovery score was examined at15 minute's intervals. The time taken to Aldrete score $\geq$9 were noted. Patients were discharged to ward when they fulfilled 10 points from Aldrete score. Patients were questioned at postoperative day 1 for intraoperative recall. ANOVA and $x^2$ test were used for statistical analysis. RESULTS: Sixty patients were enrolled the study. There was no difference in demographic data and anesthesia-surgery times. In Group II the dosage adjustment was more frequent (26% versus 3%). The patients in Group I achieved 9 points from Aldrete score faster than Group II (5±2 versus 8±2 minute). Intraoperative recall incidence was lower in Group I. CONCLUSION: Midazolam or propophol infusion after remifentanil bolus provides adequate sedation for cystoscopic procedures. Midazolam combination may be preferable for rapid recovery profile and lower incidence of recall.

Özet

AMAÇ: Bu çalışmanın amacı tanısal sistoskopi girişimlerinde midazolam yada propofol infüzyonuna ek olarak bolus remifentanil uygulamasının sedasyon ve analjezi üzerine etkisi ile derlenme karakterlerinin incelenmesidir. GEREÇ VE YÖNTEM: Altmış hasta çalışmaya kabul edildi. Gerekli monitorizasyon sonrası 0,5 mcg.kg-1 bolus remifentanil uygulandı. Grup I'e 1 mcg.kg-1.dak-1 midazolam, Grup II'ye ise 50 mcg. kg-1.dak-1 dozdan propofol infüzyonu yapıldı. Kalp hızı, ortalama kan basıncı ve periferik kandaki hemoglobin doygunluğu işlem boyunca not edildi. Hastaların sedasyon seviyelerine göre ilaç infüzyon dozları ayarlandı. Anestezi ve cerrahi süreleri not edildi. İşlem sonrası Aldrete derlenme skoru 15 dakikalık dönemlerde incelendi. Hastaların Aldrete derlenme skoru $\geq$9 olduğu an kayıt edildi. Aldrete derlenme skoru 10 olduğu an hastalar odalarına gönderildi. Grup I'deki hastalarda Aldrete derlenme skoru 9? puan sonucu daha kısa süre içinde elde edildi (5±2, 8±2 dakika).
