Abstract

Objective: Mortality following colorectal surgery decreased dramatically in recent years despite morbidity still remains at the same rate. Material and Methods: In this retrospective study, factors affecting morbidity and mortality were investigated in 58 patients who were operated for colorectal cancer at Numune Searching and Education Hospital in a 6-year period. The statistical relations were analysed between risk factors including age, comorbid diseases, preoperative bowel cleaning, localization and stage of the tumor, antibiotherapy, performed surgical procedures, and morbidity and mortality by using chi square and Mann Whitney U tests. The most performed procedures were abdominoperineal resection (Miles), right hemicolectomy and Hartman procedures. Presacral hemorrhage (19.4%) was the most common perioperative complication following abdominoperineal resection procedure (p<0.05), where as wound infection in the postoperative period (16%) (p<0.05). Comorbid diseases had significant effect on morbidity (p<0.05). Results: Total morbidity was 50% and mortality was 8.8%. Presacral bleeding and perineal infection were more common in patients treated with abdominoperineal resection evaluated with chi-square test (p<0.05), where as mortality was higher in patients with co-morbid diseases evaluated with Mann-Whitney U test. Conclusion: Careful preoperative assessment, correction of comorbid diseases like pulmonary and nutritional deficiencies, avoidance of emergency procedures if possible, performing sharp dissection between fascia propria and sacrum named 'kutsal plan' and meticulous postoperative monitorization in the intensive care unit may improve the morbidity and mortality rates.
