Abstract

Objective: Tamoxifen, a widely used drug for treatment of breast cancer, causes proliferative changes in endometrium ranging from endometrial hyperplasia to endometrium cancer. We aimed to evaluate the efficacy of Levanorgestrel releasing intra uterine devices (IUD) in preventing these changes in endometrium caused by tamoxifen. Material and Method: Postmenopausal patients taking tamoxifen at least for one year were evaluated in this randomised prospective study. All patients had undergone gynecologic examination, ultrasound, office hysterescopy and full curettage and histologic work-up and randomised to study or placebo groups. Levanorgestrel-releasing IUD was applied to patients in study group after taking informed constents. Following one year, Levanorgestrel-releasing IUD's were removed and patients were re-evaluated. Results were compared with control group. Results: The results of initial histerescopy and histology were similar in 24 patients forming the study group and 29 patients forming the placebo group. After one year, polip developed in one patient in study group and in 5 patients in placebo group. Hystologic material was desidualization in 21 (%87.7) patients in the study group. Patients in the study group had more vaginal bleeding compared with the placebo group. Conclusion: Results of our study shows that Levanorgestrel-releasing IUD prevents endometrium from proliferative effects of tamoxifen. Further studies are needed to confirm the clinic usage of Levanorgestrel- releasing IUD for postmenopausal women using tamoxifen.
