Abstract

Objective: Pleural effusion is a life-threatening condition in childhood. The aim of this study was to evaluate characteristics of children with pleural effusion. Material and Methods: The medical records of 34 children who had been hospitalized for pleural effusion, between January 1999 and April 2002 were evaluated, retrospectively. Age, gender, clinical and laboratory signs, culture results, complications, and management were recorded. Results: Nineteen of the cases (55.9%) were boys, 15 of the cases (44.1%) were girls and their mean age was 5.2±3.7 (1-14) years. The most common clinical findings were fever, cough and dyspnea in admission. Thoracentesis was applied to the 30 cases. The characteristic of pleural fluid of 21 cases (61.8%) was exudate, while pleural fluid of 9 cases (26.5%) was transudate. Bacteriological cultures were positive in 11 cases (32.4%) and the most commonly detected microorganism was Streptococcus pneumoniae (17.6%). Antibiotics were given to all patients and thorax tube and intrapleural streptokinase were used 23 patients (67.6%). The most common complication was pleural thickening. Conclusion: This study presents the characteristics of patients with pleural effusion in our region and guide to perform new studies, which aim to find risk factor of this disease.

Özet

Amaç: Parapnömonik efüzyon çocukluk çağında hayatı tehdit eden bir durumdur. Bu çalışmanın amacı parapnömonik efüzyonla başvuran çocukların özelliklerini değerlendirmektir. Gereç ve Yöntem: Ocak 1999-Nisan 2002 tarihleri arasında parapnömonik efüzyon tanısıyla hastaneye yatırılan 34 çocuğun dosya kayıtları incelenerek yaş, cinsiyet, klinik ve laboratuar bulguları, kültür sonuçları, komplikasyonlar ve tedavi yönetimi incelendi. Bulgular: Olguların 19'u erkek (%55.9), 15'i kızdı (%44.1) ve ortalama yaşları 5.2±3.7 (1-14) yıldı. Başvuruda en sık gözlenen klinik bulgular ateş, öksürük ve dispneydi. Torasentez 30 olguya yapılmıştı. Efüzyonun karekteri; 21 olguda (%61.8) eksuda, 9 olguda (%26.5) transuda idi. Bakteriolojik kültürler tüm olguların 11'inde (%32.4) pozitifti ve en sık tespit edilen mikroorganizma Streptococcus pneumoniae (%17.6) idi. Tüm olgulara antibiyotik verildi. Olguların 23'üne (%67.6) toraks tüpü ve intraplevral streptokinaz uygulandı. Olgularda en sık tespit edilen komplikasyon plevral kalınlaşmaydı. Sonuç: Bu çalışma bölgemizde saptanan parapnömonik efüzyonlu olguların özelliklerini yansıtmakta olup, bu hastalığın risk faktörlerinin belirlenmesine yönelik yapılacak çalışmalara ışık tutacaktır.
