Abstract
In our study we compared the effects of mivacurium, which is a short lasting non-depolarizting muscule relaseant, in two different ways of administration, such as bolus and fractioned closes. 34 patients, aged 2-12, belonging to the I-II ASA categories, scheduled for elective surgery were randomly allocated in two groups. In group I, mivacurium was given in a single dose (0,25 mg/kg), in group II infractioned dose (0,15 mg/kg bolus, after 30 seconds 0,10 mg/kg). General anesthesia was induced with thiopenthal sodium 5 mg/kg and mivacurium. Anesthesia was maintained with sevoflurone 1,5-2% in combination with nitrous oxide (N20) 60% in oxygene. We observed that compared with single bolus dose, mivaurium, infractioned doses has better effects on the quality of tracheal intubation, skin reactions were rare, time of action was short. We conclude that because of these adventages mivacarium infractioned doses, in children who don't need crush intubation, is superior to the single bolus dose in short-lasting surgical procedures.

Çalışmamızda çocuklarda kısa etki süreli nondepolarizan noromüsküler bloken olan mivakuryumu, bolus ve bölünmüş dozda olmak üzere iki farklı şekilde vererek etkilerini karşılaştırdık. Çalışmamız ASA sınıflamasına göre I-II gruplarına dahil, yaşları 2-12 arasındaki, kilo ve boy açısından normal sınırlarda kabul edilen elektif cerrahi girişim yapılacak 34 hastaya uygulandı. Hastalar rastlantısal olarak her birinde 17'şer hasta olan 21 gruba ayrıldı. Grup I'de mivakuryum (0.25mg/kg) bolus tek doz, GRUP II'de mivakuryum (0.15 mg/kg'dan 30sn sonra 0.10 mg/kg) bölünmüş doz olarak mivakuryum ile sağlandıktan sonra, idame %40 oksijen %60 Azotprotoksit ve % 1.5-2 sevoflorane ile sürdürüldü. Mivakuryumun bölünmüş dozda kullanımının bolus doza göre entübasyon kalitesinin daha iyi olması, cilt reaksiyonlarının daha az görülmesi ve etki süresinin daha kısa olması gibi üstünlükleri nedeniyle çocuklarda hızlı entübasyon gerekmeyen kısa süreli girişimlerde kullanılması avantaj yaratacağı sonucuna varılmıştır.
