Abstract

BACKGROUND: To assess with Doppler colour flow imaging whether a brain-sparing effect is present 3tid in all cerebral arteries of the circle of Willis in the small for gestational age ( SGA) fetus aVid to rod establish the sensitivity of the pulsatility index ( PI) in these vessels and in the umbilical artery in the identification of SGA fetuses. R.ev METHODS: Forty-one SGA fetuses and 54 appropriate for gestational age fetuses (AGA) at 24 to 38 weeks of gestation were studied. The middle cerebral artery (MCA), internal carotid artery (ICA), mal anterior cerebral artery (ACA) and posterior cerebral artery (PCA) were studied in all fetuses with ıred Doppler color flow imaging for calculation of the pulsatility index. The pulsatility index of fetal gin. jntracerebral arteries and umbilical artery ( UA) were compared between SGA fetuses and controls. !' RESULTS: Acceptable flow velocity waveforms were obtained in the intracerebral circulation in more ects than 82 % . In SGA fetuses, pulsatility index in all intracerebral arteries were significantly reduced and: compared with normal pregnancy, suggesting participation of all major intracranial arteries in a brain-itro. ■ sparing effect in the presence of chronic fetal hypoxia. The pulsatility index of the umbilical artery and middle cerebral artery are the most discriminative factors between SGA fetuses and controls. JK, CONCLUSION: Doppler color flow imaging allows easy identification of fetal intracerebral arterial y on vasculature. SGA fetuses with an abnormal UA PI have cerebral-sparing effect as suggested by a drop and in cerebral vascular resistance. MCA and UA PI values seems to be the best parameters for n.
