Abstract

This report presents a case with adnexial torsion due to a primary ovarian lipoleiomyoma. The symptoms, signs, diagnosis, treatment and histopathological features are presented and discussed according to the review of the literature. A 50-year-old, postmenopausal woman, gravida 6, para 2, abortus 4, presented to Ankara University Faculty of Medicine, Department of Obstetrics and Gynecology with a sudden onset, severe abdominal pain. Her history was otherwise unremarkable. On examination, there was direct and rebound tenderness in the left lower quadrant of the abdomen. The gynecological examination revealed a tender, solid mass of approximately 10 cm size in the left adnexial region. The mass was seperate from the uterus and mobile. The transvaginal ultrasqnography confirmed the presence of a well-circumscribed solid mass with heterogeous echogenic pattern, measuring 10 x 10x8 cm, located in the left adnexial region. Both ovaries could not be observed as different structures and the borders of the mass were clearly differentiated from those of the uterus. Uterus was measuring 6x4x3 cm and myometrial echogenicity was heterogenous. Endometrial thickness was 4 mm. Complete blood count and blood chemistry findings were normal. Tumor markers were all in the normal range (AFP: 0.62 lu/ml, CA 19.9: 17.9 u/ml, CA-125: 14.9 u/ml, CA 15.3: 46.3 U/ml, CEA: 1.7 ng/ml) The patient then underwent laparotomy. Through 15 cm
