OBJECTIVE: The aim of this study was to evaluate the value of transvaginal ultrasonography in assessment of myometrial invasion in endometrial cancer and to compare the results with histopathologic findings. STUDY DESIGN: Fifty-four patients with endometrial carcinoma underwent transvaginal ultrasonography (TVUS) and myometrial invasion studies. TVUS and myometrial invasion findings were correlated with pathological findings obtained by surgical staging. Diagnostic accuracies of detecting myometrial invasion were assessed with the use of histopathological findings as the standart. RESULTS: Evaluation the depth of myometrial invasion with transvaginal sonography was accurate in 42 out of 54 patients (accuracy 78%, sensitivity 88%, spesivity 68%, positive predictive value 70%, negative predictive value 90%). CONCLUSION: TVUS may be useful for the diagnosis of myometrial invasion in endometrial cancer. This noninvasive method can consequently be regarded as an important tool in the establishment of individualised treatment program in case of women with endometrial cancer.

Bu çalışmanın amacı endometrium kanserinde myometrial invazyonun tespitinde transvaginal ultrasonografinin (TVUS) etkinliğini değerlendirmek ve sonuçları histopatolojik bulgularla karşılaştırmaktır. Hastanemizde preoperatif endometrium kanseri tanısı almış 54 olguya TVUS ile myometrial invazyon tespiti çalışması yapılmıştır. Bulgular cerrahi evreleme sonucu elde edilen patolojik verilerle karşılaştırılmıştır. Myometrial invazyon tespitindeki tanısal doğruluk, histopatolojik bulgular standart alınarak değerlendirilmiştir. 54 olgunun 42'sinde TVUS ile doğru değerlendirme yapılmıştır. İstatistiksel olarak; %78 doğruluk, %88 duyarlılık, %68 özgünlük, % 70 pozitif belirleyici değer, %90 negatif belirleyici değer bulunmuştur. Sonuç olarak endometrium kanserlerinde myometrial invazyon varlığının saptanmasında TVUS yararlı bir preoperatif yöntem olmakla beraber, bu noninvaziv metodun endometrium kanserli hastalarda kişisel tedavi programlarının belirlenmesinde de önemli bir yer alabileceği düşüncesindeyiz.
