Abstract

OBJECTIVE: To determine the neonatal acid-base status in patients with abnormal fetal heart rate (FHR) tracing patterns in labor. STUDY DESIGN: A prospective study was conducted with 89 patients with normal and 71 patients with pathologic FHR patterns, all in labor at term. Informations regarding maternal age, gravidity, parity, gestational ages were obtained and fetal heart recordings of at least 30 minutes were obtained in all patients using an external monitor at admission and during the first stage of labor. Umbilical artery cord blood samples were collected immediately after delivery. Apgar scores at 1 and 5 minutes, mode of delivery and intensive care unit admission of the neonates were recorded. Fetal acidosis was defined as pH values <7.20. Statistical analyses were performed and significance was set at p<0.05. RESULTS: The rate of fetal acidosis was 41% in patients with abnormal and 1.1% in patients with normal FHR patterns. Abnormal FHR tracings present in first stage of labor was statistically significantly associated with fetal acidosis, cesarean delivery and increased neonatal care unit admission rates (p<0.0001). Both 1 and 5 minute Apgar scores were significantly lower in abnormal fetal heart rate tracing group when compared to normal fetal heart rate tracing group (p<0.0001). CONCLUSION: For the prediction and diagnosis of asphyxia electronic fetal monitoring has clinical value since the rate of fetal acidosis is increased in patients with abnormal fetal heart rate patterns.
