Abstract

Objective: Pressure ulcer is a frequent complication in critically ill patients. We aimed to investigate pressure ulcer incidence and risk factors in the intensive care unit (ICU). Methods: Following Ethics Committee approval, we retrospectively analyzed the data from all adult patients with pressure ulcers in the ICU from January 1 to December 31, 2006. Age, gender, diagnosis, concomittant disease, morbidity, reposition, use of sedatives/analgesics/neuromuscular blocking drugs, duration of mechanical ventilation (MV), PaO2/FiO2 ratio, albumin level, length of stay (LOS) in ICU, outcome, onset date and location and level of pressure ulcer were recorded. Results: There were 74 patients enrolled in this study. The mean age of the patients was 46.0 ± 18.2 years, 45 of them were male and 29 were female. Pressure ulcer was developed in 26 (35.1%) patients. The most common beginning location was sacrum (61.5%) with grade I (86.4%). During treatment in ICU, pressure ulcer sites were the sacrum (53.3%), gluteal region (13.3%) and heels (13:3%), and the most common grade was II (61.5%). Pressure ulcers were related to limited reposition (p<0.05), duration of MV (p<0.01) and low PaO2/FiO2 ratio (p<0.01). The mean ICU stay was 27.46±32.01 days. Prolonged LOS in the ICU was significantly related to pressure ulcer (p<0.001). During the study period, 47 patients (63.5%) survived, while 27 (36.5%) died. Conclusion: We determined that limited reposition, low PaO2/FiO2 ratio, prolonged MV and LOS in ICU were significantly associated with the presence of pressure ulcers.
