Abstract

Objective: In this study we aimed to investigate the species and the antibiotic susceptibilities of the microorganisms isolated from patients who had been treated in intensive care units between January 1997 and January 2002, and to determine the changes in the causative microorganisms and their antibiotic resistance in time. Material and method: In this study, microorganisms recovered from different specimens of patients who were hospitalized in the ICU's of Dokuz Eylül University hospital between 1.1.1997 and 1.1:2002 were reviewed retrospectively for the distrubition of different species among the specimens and antibacterial susceptibility patterns. Results: During this period, the number of isolates from respiratory tract specimens seems to be gradually decreasing, whereas those of blood cultures have been increasing. The study revealed that, Gram negative non-fermentative bacteria in respiratory samples, Gram negative non-fermentative bacteria and Staphylococcus aureus in wound samples, coagulase negative staphylococci (CNS) in blood samples, and Candida spp. and Esherichia coli isolates in urinary samples were the most frequently isolated organisms overall. Candida species showed a significant pre-valance in 2000, and it made up the 20% of all isolates. When the antibiotic resistance patterns of the ICU isolates are reviewed, it was observed that car-bapenem resistance among Gram negative non fermentative bacteria was %18, which increased to %57 in 1999, and stayed around this figure for the last two years. The Gram negative bacteria were mostly multiresistant and a significant number of isolates which are resistant to all antibiotic groups were recovered. S. aureus and CNS, the leading Gram positive bacteria, had high methicillin resistance during all five years. Conclusion: Our results suggest that every effort should be made in choosing the empirical antibiotic regimen for ICU infections and also the education of the staff bears sustained importance to control the spread of resistant microorganisms.
