Abstract

The aim of this study is to compare the effectiveness, safety and tolerance of a new macrolide antibiotic, clarithromycin, and the traditional drug amoxicillin at the therapy of sinusitis which is one of the most common childhood diseases. Hundred patients (50 girls, 50 boys, between 3.5 and 13 age) with signs of acute bacterial sinusitis and positive radiological findings are included in this study between September 2002 and February 2003 at outpatient clinic of SSK Bakırköy Maternity and Children's Hospital. 50 of the patients were given amoxicillin (40 mg/kg/day) three times daily for 10 days and the other 50 of them were given clarithromycin (15 mg/kg/day) twice daily for 10 days. The patients were asked to return to the clinic for control 11 days after their first visit. At the control; symptoms were questioned, detailed physical examinations was performed and radiographic examinations and laboratory tests were repeated. Patients w clinical response at day 11 were asked to return for a control at day 28. Physical examinations and Water's performed and symptoms were questioned. In our study improvement was noted in 88 % of amoxicillin recipients and 86 % of clarithromycin recipients control at the 28th day revealed 14 % relapse in the amoxicillin group and 8 % in the clarithromycin group (p=0.611. were well tolerated. The adverse effect rate was 6 %for amoxicillin and 12 %for clarithromycin. In both group moderate gastrointestinal adverse effects were reported and no patient was withdrawn from treatment due to the at According to our results, 10 days course of standard dose amoxicillin given three times a day is as effect well-tolerated as a 10 days course of clarithromycin given twice a day in children with acute sinusitis and Wt that amoxicillin may be regarded as the first choice drug in noncomplicated cases for economical reasons.
