Abstract

Objective: To evaluate risk factors, clinical charac-teristics, laboratory, and microbiological character-istics and echocardiographic features of our patients with infective endocarditis (IE).Material and Methods: Between the years 2004-2011, according to the Duke and modified Duke criteria, 36 patients were diagnosed with definite or probable infective endocarditis. The clinical, labora-tory, microbiological, medical and surgical therapy results and ECHO properties were evaluated retro-spectively.Results: According to modified Duke criteria, 36 patients were diagnosed with definite or possible IE; 21 were girls (58.3%) and 15 were boys (41.7%) with a median age of 7 years (3 months-18 years) and median body weight of 18.2 kg (3.2-55 kg). There were 14 cases (38.9%) of definite IE, and 22 patients (61.1%) had possible IE. Fourteen cases (38.9%) had positive blood cultures. The most frequently isolated organism was Streptococcus viridans, fol-lowed by Staphylococcus aureus and Staphylococcus epidermidis. Thirty-two cases (88.9%) had a risk factor. In 22 of them (61.1%), congenital heart disease (CHD) was present as a risk factor. Vegetation was seen by ECHO in 35 cases (97.2%). Twenty-four cases (66.7%) had a high fever over 38.3°C. Thirty-two cases (88.9%) had murmur, 21 cases (58.3%) had splenomegaly, 19 cases (52.8%) had HM, 10 cases (28%) had hematuria, and four patients (11.1%) had arthritis. Ten cases (28%) had congestive heart failure. Complications related to IE occurred in a total six patients (16.6%). Conclusion: Currently, the cause for the increase in the frequency of IE in children is thought to be pro-longed survival of patients with CHD, more frequent use of invasive procedures and prosthetic material, frequent use of intravascular catheters in hospital-ized newborns and infants, and the increased num-ber of oncology patients receiving long-term che motherapy. Good knowledge of the risk factors of IE, in particular high fever in patients with congenital heart disease, will help in the diagnosis of IE.
