Abstract

In this study, postoperative analgesic effects of intravenous paracetamol administration in lumbar discectomy patients were evaluated. After the approval of ethic committee, 90 patients undergoing lumbar disc hernia operation randomly divided into 3 groups. After standart general anesthesia, patients in group I received 1 gr i.v. paracetamol infusion 15 minutes before the induction, patients in group II received i.v. paracetamol infusion started 15 minutes before the end of surgery. i.v morphine via PCA is used for postoperative analgesia maintenance and patients pain scores were assessed with VAS at 0., 1., 2., 3., 6., 12. and 24. hours. First analgesic requirement time, total morphine consumption and side effects were recorded. In group I and II, VAS scores, 24 h morphine consumption and first morphine requirement times were significantly different comparing to group III. As a result, we think that in lumbar discectomy cases preoperative administration of 1 gr paracetamol has no preemptive analgesic effect.

Özet

Çalışmamızda intravenöz parasetamolün preemptif uygulamasının lomber diskektomi hastalarında post operatif ağrıya olan etkisi araştırıldı. Lomber disk hernisi nedeniyle opere edilecek 90 hasta, etik komite izni ile çalışmaya alınarak rastgele üç gruba ayrıldı. Standart genel anestezi uygulanan tüm hastalardan Grup I’deki hastalara (n=30) anestezi indüksiyonunundan 15 dakika önce 1 gr parasetamol iv infüzyon, Grup II’deki hastalara (n=30) ise 1 gr parasetamol iv operasyon bitiminden 15 dakika önce infüzyon şeklinde uygulandı. Grup III ( n=30 ) ise kontrol grubuydu. Post operatif analjezileri i.v. morfin PCA ile sağlanan hastaların ağrıları 0.,1.,2.,3.,6.,12.,24. saatlerde Vizüel Analog Skala ( VAS ) ile değerlendirildi. İlk analjezik istem zamanı, kullanılan toplam morfin miktarı ve yan etkiler kaydedildi. Grup I ve II’nin VAS değerleri, 24 saatlik toplam morfin kullanımı ve ilk morfin istem zamanları Grup III’den anlamlı olarak farklı bulunmuştur( p<0.001). Sonuç olarak, lomber diskektomi olgularında 1 gram parasetamolün preoperatif uygulamasının preemptif analjezik etkinliği olmadığı kanaatindeyiz.
