Abstract

Purpose: Carpal tunnel syndrome (CTS) is caused by the decompression of the median nerve as it transits the carpal tunnel. If surgery is needed, compression of the median nerve is removed by open carpal tunnel release (OCTR). The aim of this randomized prospective study is to determine the effectiveness of median nerve and tendon gliding exercises following OCRT. Material and methods: Forty-three patients (40 female, 3 male) who had unilateral OCRT, with a mean age of 46.07 ± 7.63 years were enrolled to the study. All the patients were evaluated preoperatively with clinical and electrophysiological parameters. The patients were randomly assigned into two groups as exercise and control groups. Postoperatively 21 patients in the exercise group began nervous medianus and tendon gliding exercises at 3rd week. Patients were evaluated with Boston Questionnaire (BQ), grip strength, lateral, tip and three-fingered pinch measurements, 2-point discrimination (2- PD), Semmes Weinstein Monofilament Test (SWMT) and Nine Hole Peg Test (NHPT) and electrophysiological parameters at 3rd and 6th months after the operation. Results: At 3rd and 6th months control both of the groups had significant improvements in BQ, NHPT and median motor nerve distal latencies. At 6th months control both of the groups had statistically significant improvements in palm-wrist median sensory nerve velocity and action potential amplitude. Conclusion: In the follow-up period of OCTR, performing nervous medianus and tendon gliding exercises did not provide benefit in the recovery of sensorial and motor functions of the patients.

6. ay kontrollerinde avuç içi-bilek segmenti median duyusal sinir iletim hızı ve aksiyon potansiyeli amplitünde her iki grupta istatistiksel anlamlı düzeyde artış saptandı. Sonuç: AKTS operasyonu sonrasında takip döneminde median sinir ve tendon kaydırma egzersizleri yapılmasının hastaların duyusal ve motor fonksiyonlarına katkı sağlamadığı saptanmıştır.
